# Create Form Template

<mark style="color:orange;">**Creating a Form Template**</mark>

To define the structure and required fields for the information needed from vendors.

Navigate to the <mark style="color:blue;">**Policy Management**</mark> section in the <mark style="color:blue;">**ComplianceBot dashboard**</mark>.

Click on <mark style="color:blue;">**Create New Template.**</mark>

Fill in the necessary details:

* <mark style="color:blue;">**Template Title**</mark>&#x20;
* <mark style="color:blue;">**Description**</mark>
* <mark style="color:blue;">**Template option**</mark>
* <mark style="color:blue;">**Version**</mark>

Select a <mark style="color:blue;">**recurrent schedule**</mark> if needed. This will determine how often the document needs to be reviewed.

Lastly, click on the <mark style="color:blue;">**Create**</mark> button&#x20;
